    The importance given to a beautiful smile is not unique to our day. The pursuit of beauty can be seen even in ancient civilizations. Both the Phoenicians (800 BC) and the Etruscans (900 BC) carefully carved ivory to mimic the shape and tone of natural teeth. Until the 18th century, dentistry was not considered a separate discipline and it was not known that it was divided into various branches. The French pioneer of the movement, Pierre Fauchard (1678-1761), along with many of his colleagues, modernized dentistry, advanced it, and advocated aesthetic practices.

    The aim of an aesthetic renewal is to develop a seamless chewing system in which teeth, tissues, muscles, skeletal structures and joints all work in harmony. When planning the treatment for aesthetic cases, smile design is too important to be separated from a comprehensive patient treatment approach. Achieving a successful, healthy and functional outcome requires understanding the relationship between the muscles, bones, joints, gingival tissues and all of the supporting oral structure, including occlusion.

      What are the components of an Aesthetic Smile?

      An aesthetic smile requires the perfect integration of both face and tooth composition. The facial composition includes the hard and soft tissues of the face. Tooth composition more specifically includes the teeth and their relationship to the gingival tissues. A smile design should always include the evaluation and analysis of both the face and tooth composition. 

      • When viewed from the front, the basic shape of the face can be one of the following:
      • Square – Narrowing – Narrowing square – Oval
      • When viewed from the side, a person’s profile can be any of the following:
      • Straight – Convex – Concave

      These factors play a role in determining the size, shape, and side profile of the tooth. In short, tooth morphology depends on the facial morphology.

      TEETH-RELATED POINTS

      Teeth visibility

      When the mouth is opened comfortably and slightly, the incisal third 3.5 mm of the maxillary middle anterior tooth should be visible in a young individual. With age, the decrease in muscle tone causes fewer teeth to appear. Phonetics is a key determinant of the tooth length. To determine the appropriate lip, tongue, incisor support and tooth position, the patient should sit or stand upright in the phonetic exercises.

      Thread Sizes

      Correct tooth proportion is associated with facial morphology and is important in designing an aesthetically pleasing smile. The proportions of the maxillary central incisors must be aesthetically and mathematically correct. The ratio of their widths to their lengths should be approximately 4/5 (0.8-1.0). The shape and location of the centrals also affect the appearance and placement of the lateral and canine teeth. It is necessary to reach the correct proportions in an aesthetically pleasing smile.

      The buccal (cheek) corridor is the dark space (negative space) that appears during smiling between the corners of the mouth and the cheek-facing surfaces of the upper teeth.

          SOFT TISSUE-RELATED POINTS

          Gingival Health (gum health)

          Since the gingiva functions as the framework of the teeth, the final aesthetic success of the case is significantly affected by gingival health. It is very important to fully improve the health of the gums before starting any treatment.

          Healthy gingiva usually has the following features.

          Pale pink, orange peel appearance, solid and matte

          Its facial surface is 3 mm above the level of the alveolar bone.

          It should end in the contact area 5 mm above the bone in the interdental area and fill the gingival embrasure. 

          Gingival level and harmony

          Creating the right gum levels for each tooth is the key to a harmonic smile. The gingival height (position and level) of the centrals should be symmetrical. Also, this level should match that of the canines. It is acceptable that the lateral incisors have the same gingival level, but the gingival image closer to the plane of the cutting edge makes the smile more pleasant and preferable. The least preferred form is that the lateral incisors are at the root level compared to the central and canine teeth.

          In the light of all these, the smile we create should be aesthetically appealing and functionally pleasing. It is our duty to evaluate, diagnose and get the best results by considering all the factors of our patients. We should be as conservative (protective) as possible while designing a smile. Our aim should be to achieve the most aesthetic and most durable result with the least reduction (removal) from the tooth structure. Cosmetic dentistry requires a multidisciplinary work; Orthodontic, periodontal and surgical procedures can be applied when necessary.
